Short answer: how much is an X-ray in Russia?

With an OMS policy and a doctor's referral, X-rays are free at state polyclinics and hospitals. Paying out of pocket, a single-region X-ray costs about ₽1,200–1,400 (≈$14–$16) as a paid service at a regional state hospital and roughly ₽2,000–3,600 (≈$24–$42) at Moscow private clinics. The Moscow average for a one-view chest X-ray (рентген легких) is about ₽2,400; MEDSI lists it from ₽2,900. Private CT starts around ₽6,000 and MRI around ₽7,500.

Quick Overview

Russia's public system is funded through OMS (обязательное медицинское страхование — compulsory medical insurance). Every citizen and eligible resident gets an OMS policy and attaches to a local polyclinic; imaging ordered by your doctor there is free under the annual state guarantees programme. Alongside it there are three ways to pay: paid services (платные услуги) at state hospitals, which are usually the cheapest cash option; private clinic chains such as MEDSI, SM-Clinic, Mother and Child, EMC and K+31; and DMS (voluntary private insurance), usually provided by employers. Moscow is the most expensive city; regional prices are often about half.

Premium clinics (EMC and similar) charge more.

How OMS, Paid Services and DMS Work

OMS: free imaging

  • Policy issued by an insurance medical organisation (e.g. SOGAZ-Med, Ingosstrakh-M); paper, plastic or digital record via Gosuslugi
  • Attach to one polyclinic (you can change once a year)
  • GP or specialist issues a referral; X-ray, CT and MRI are then free
  • The state guarantees programme sets maximum waits (≈14 working days for diagnostics, ≈7 if cancer is suspected)
  • If the wait is exceeded, complain to your OMS insurer — they must help arrange the test

Paid services (платные услуги)

  • State hospitals may sell services to patients who choose to pay
  • Prices are fixed in a published price list, usually well below private chains
  • Useful for people without OMS or who want a faster slot
  • You get a contract and a receipt — keep them for the tax deduction

DMS (voluntary insurance)

  • Mostly employer-paid; insurers include SOGAZ, Ingosstrakh, AlfaStrakhovanie, RESO-Garantia, Rosgosstrakh
  • Direct billing at network clinics (MEDSI, SM-Clinic and others)
  • X-rays usually included; CT/MRI may need insurer approval
  • Patent-holding migrant workers must buy a DMS policy

Where to Get an X-Ray

🏥 State hospitals and polyclinics

  • Your attached district polyclinic — the default for OMS X-rays
  • Moscow city hospitals (e.g. Sklifosovsky Research Institute for emergencies, Pirogov City Clinical Hospital No. 1)
  • Sechenov University clinics — Moscow
  • Pavlov First St. Petersburg State Medical University
  • Regional emergency hospitals (БСМП) and regional clinical hospitals

🩻 Private chains

  • MEDSI — largest private network; published Moscow prices used in the table
  • SM-Clinic — Moscow, St. Petersburg and other cities
  • Mother and Child (Мать и дитя) — hospitals and clinics in many regions
  • Invitro — best known for laboratories; some of its medical centres offer imaging
  • EMC and K+31 — premium Moscow clinics, English-speaking service

📍 Regional differences

  • Moscow private average: ≈₽2,100–2,900 per region
  • Regional state hospitals (paid): ≈₽1,200–1,400 per region
  • St. Petersburg is usually a little cheaper than Moscow
  • Small towns may lack CT/MRI — referral to a regional centre is common

FAQ

How much does an X-ray cost in Russia in 2026?

With an OMS (compulsory medical insurance) policy and a doctor's referral, X-rays at state polyclinics and hospitals are free. Paying privately, a single-region X-ray costs about ₽1,200–1,400 (about $14–$16) as a paid service at a regional state hospital and roughly ₽2,000–3,600 ($24–$42) at Moscow private clinics. The Moscow average for a one-view chest X-ray is about ₽2,400; MEDSI lists it from ₽2,900.

Сколько стоит рентген в Москве? (How much is an X-ray in Moscow?)

Aggregated Moscow clinic prices put a one-view chest X-ray at about ₽2,380 on average and a two-view study at about ₽2,620, with lumbar spine around ₽2,850 and pelvis around ₽2,300. Large chains cost more: MEDSI lists chest X-ray from ₽2,900 (one view) or ₽3,600 (two views) and most joints and spine from ₽3,000–3,300.

Is an X-ray free under OMS (рентген по ОМС)?

Yes. X-rays, ultrasound, CT and MRI are part of the state guarantees programme funded through OMS, so they are free when ordered by your doctor at the polyclinic you are attached to. You need your passport, OMS policy (paper, plastic or the digital record) and a referral (направление). Without a referral you will usually be asked to pay.

How long is the wait for an X-ray or CT under OMS?

The federal state guarantees programme caps waiting times for diagnostic instrumental tests, including X-ray, CT and MRI, at roughly 14 working days from the referral, and shorter (about 7 working days) when cancer is suspected. Regional programmes can set shorter limits. Many people who need a result the same day pay for a private X-ray instead.

Is fluorography (флюорография) free?

Yes. Screening fluorography for tuberculosis is free under OMS and is included in routine health checks (диспансеризация). Adults are generally advised to have it at least once every two years. Employers, universities and migration services often require a recent result.

Can foreigners get free X-rays in Russia?

Foreigners with a permanent or temporary residence permit, refugees, and EAEU citizens (Armenia, Belarus, Kazakhstan, Kyrgyzstan) working under an employment contract can obtain an OMS policy. Other foreign employees generally qualify after three years of employer insurance contributions. Tourists and most patent workers are not covered and pay private rates or use their travel or DMS insurance; emergency care is provided free to everyone.

What is DMS and does it cover X-rays?

DMS (добровольное медицинское страхование) is voluntary private health insurance, often provided by employers through insurers such as SOGAZ, Ingosstrakh, AlfaStrakhovanie, RESO-Garantia and Rosgosstrakh. Most policies include diagnostic X-rays at network clinics such as MEDSI or SM-Clinic with direct billing, but check your programme's list of services and clinics.

How much does a CT or MRI cost in Russia?

Free under OMS with a referral. Privately in Moscow, MEDSI lists CT from about ₽6,000–6,400 (brain or spine) to ₽8,600–8,800 (abdomen or chest), and MRI from ₽7,500 (brain or lumbar spine) and ₽10,700 (knee). Contrast is often charged separately.

Can I get a tax refund for a paid X-ray?

Russian tax residents can claim the social tax deduction for medical services: 13% of what they paid, within an annual limit of ₽150,000 for ordinary services (the limit applies from 2024). You need a certificate of payment for medical services from the clinic, and the clinic must hold a Russian medical licence.

Are X-rays cheaper outside Moscow?

Usually. A state emergency hospital in Krasnoyarsk, for example, lists paid X-rays of the knee, ankle, wrist or cervical spine at ₽1,200 and digital chest X-ray, lumbar spine or pelvis at ₽1,400 (May 2026 price list) — roughly half the Moscow average. Private clinics in regional cities typically fall between the two.
